Self referral & Self Help Services for patients

Third Sector Services

Sometimes patients need social support rather than medical support but don't know where to go to get this help.  Due to the ongoing increase in demand to see a GP it is critical that we do everything we can to help our patients to be able to access services directly without having to wait to see a GP first.

About EPP Cymru Education Programmes for Patients (EPP Cymru) provide a range of self-management courses for people living with a long-term health condition/s and carers.
Our self-management courses support you to have the confidence, information and skills needed to help you manage a condition in partnership with your health andsocial care professionals.

 Long COVID Service Self-Referral - Betsi Cadwaladr University Health Board 

If you have any of the following symptoms, please ensure you have seen your GP before referring yourself to the Long COVID Service

To refer yourself, it is important that you complete ALL sections of the form clearly.  All information you provide will remain confidential and stored securely in line with Data Protection legislation and the organisation’s Information Governance policies and procedures. By completing this referral you consent for the Health Board to share necessary and relevant information with your GP or other appropriate healthcare professionals.

You can now refer yourself directly to the BCUHB Long COVID Service. Alternatively, you can request to be referred by your GP or health professional.  

Long COVID Service Self-Referral - Betsi Cadwaladr University Health Board

Symptoms of Long COVID include but are not limited to:

  • Fatigue
  • Breathlessness
  • Problems with memory and concentration (brain fog)
  • Mood changes (anxiety or depression)
  • Chest Pain
  • Heart Palpitations
  • Dizziness
  • Pain
  • High Temperature

Veterans' NHS Wales

We offer psychological therapy to veterans, reservists and serving personnel awaiting discharge (with a set date of discharge) who have a mental health problem directly related to their military service. This may include trauma symptoms related to active service. We will see veterans with issues related to bullying or assault within the military. We may see veterans who have difficulties related to RTA’s or work place or training accidents related to their role within the military. We may also see veterans who were unwillingly discharged from the military and find civilian life deeply unsatisfying and have become depressed.

Since April 2017 we have designated Change Step peer mentors attached to the service. Their role is to engage the veteran, assist with any social needs and to support them in improving their level of coping and functioning.   For more information click the link Veterans Wales
